6. ราคาแพ็กเกจนี้ไม่รวมค่าตรวจอื่น ๆ ค่ารักษาพยาบาล หรือค่าบริการปรึกษาทางการแพทย์เพิ่มเติมกรณีพบความผิดปกติ
7. ชุดตรวจสุขภาพ ไม่รวมค่ารักษาหรือค่ายาเพิ่มเติมในกรณีที่แพทย์สั่ง

10. ราคานี้สำหรับคนไทยและ Expat เท่านั้น

วันหมดอายุ E-Coupon 
1. E-Coupon มีอายุการใช้งานภายในวันที่ 31 ธันวาคม พ.ศ. 2567 
2. หาก E-Coupon หมดอายุ รพ.ขอสงวนสิทธิ์การเข้าใช้บริการ และ E-Coupon ไม่สามารถขยายระยะเวลาในการใช้บริการหรือคืนเป็นเงินได้
3. แพ็กเกจราคานี้ สามารถสั่งซื้อได้ถึง 15 พฤศจิกายน พ.ศ. 2567 เท่านั้น

หมายเหตุ
1. กรณีลูกค้ามีการขอยกเลิกคำสั่งซื้อหลังสิ้นสุด Shopee Guarantee โดยไม่ได้มีสาเหตุมาจากทางโรงพยาบาล โรงพยาบาลขอสงวนสิทธิ์ในการหักค่าดำเนินการ 10% จากราคาสุทธิในใบเสร็จ
2. ยอดเงินที่แสดงใบเสร็จเป็นยอดเงินที่ได้รับจากการชำระเงินจริงไม่รวมโปรโมชั่นส่วนลด

1. This service is reserved for those who present an E-Coupon only.
2. This E-Coupon cannot be exchanged for cash.
3. E-Coupon is only applied to specific items as mentioned and changes cannot be made.
4.These packages are for outpatients who are physically dependent. 
5.These packages exclude doctor fees in case additional specialists are required.
6. The price excludes additional tests investigations or medical consultation in case of positive findings. 
7.These packages exclude treatment fees or medications prescribed by the doctors if any. 
8. Further information or to make an appointment please contact the Health Design Center at 02-3103284 02-7551591 (operating hours: 07:00 15:00) or the Contact Center at 1719.
9. Services can be received at the Health Design Center 5th Floor Building R Bangkok Hospital (Soi Soonvijai) 
10. The price is applicable to both Thai nationals and expatriates.

Expiration Date of E-Coupon:
1. The E-Coupon is valid until December 31 2024.
2. If the E-Coupon expires the hospital reserves the right to deny service access and the E-Coupon cannot be extended or refunded.
3.This health check package can be ordered until November 15 2024

Note:
1. In case customers request cancellation of the order after the Shopee Guarantee period without valid reasons from the hospital the hospital reserves the right to deduct a 10% processing fee from the net amount on the receipt.
2. The receipt amount displayed is the actual payment received excluding any promotional discounts.
